About

Berberine is an isoquinoline alkaloid found in several plants including goldenseal, Oregon grape, and barberry. It activates the AMPK pathway in enterocytes and exhibits direct antimicrobial activity against various enteric pathogens. Research suggests berberine may help modulate gut microbiome composition, including supporting the growth of Akkermansia muciniphila, a keystone species for gut barrier health. It is increasingly studied as part of herbal antimicrobial protocols for SIBO.

How It Works

Activates AMPK pathway in enterocytes modulating cellular energy metabolism; directly antimicrobial against enteric pathogens (E. coli, Staphylococcus, Candida); modulates gut microbiome composition increasing Akkermansia muciniphila; enhances intestinal barrier via tight junction protein upregulation.

Evidence For Conditions

Side Effects

  • GI side effects (diarrhea, constipation, flatulence, abdominal pain) — most common
  • May cause uterine contractions (contraindicated in pregnancy)
  • Headache
  • Skin rash (rare)

Drug & Supplement Interactions

  • Diabetes medications (berberine significantly lowers blood sugar; risk of hypoglycemia)
  • Cyclosporine (berberine inhibits CYP3A4, increasing cyclosporine levels)
  • Anticoagulants (may have additive blood-thinning effects)
  • Metformin (additive glucose-lowering effect; monitor closely)
  • Macrolide antibiotics (berberine inhibits CYP3A4)
