About

Pancreatic enzyme replacement therapy (PERT) is a combination of lipase, protease, and amylase that supports digestion when the pancreas cannot produce sufficient enzymes. It is the standard of care for exocrine pancreatic insufficiency (EPI) associated with chronic pancreatitis, cystic fibrosis, and pancreatic surgery. Research demonstrates that PERT effectively normalizes fat absorption and may help eliminate steatorrhea.

How It Works

Combination of lipase (hydrolyzes dietary triglycerides into fatty acids and glycerol), protease (breaks down dietary proteins into peptides and amino acids), and amylase (digests starches into maltose and dextrins); replaces deficient pancreatic secretion normalizing nutrient absorption.

Side Effects

  • Abdominal pain and cramping
  • Nausea
  • Fibrosing colonopathy at very high doses (rare; primarily in cystic fibrosis patients)
  • Hyperuricemia (high doses of protease can increase uric acid)

Drug & Supplement Interactions

  • Calcium carbonate and magnesium hydroxide antacids (may reduce enzyme activity)
  • Acarbose and miglitol (pancreatic amylase may counteract alpha-glucosidase inhibitors)
  • Iron supplements (protease may reduce iron absorption)
  • Proton pump inhibitors (may improve enzyme efficacy by reducing acid denaturation)
